Partial extraction therapy (PET) consists of socket shield, pontic shield and root submergence. It is a suite of techniques that emerged to combat the buccopalatal collapse that can be seen in traditional dental implant restorations over time. The basis of the procedure is leaving a buccal fragment or a subcrestal root portion attached to the buccal plate of bone. The success of the technique is due to the maintenance of the periodontal ligament resulting in nourishment and preservation of the bundle bone.

The utilization of digital technology in PET, whether it’s abutment design or crown fabrication, can play a major role in the success and minimization of complications. The abutment design must respect biological principles and the crown must address both critical and subcritical contours. Suboptimal prosthetics can lead to internal/external exposure of the shield and possible failure.

A series of representative cases ranging from single tooth to full arch will be discussed with 1 to 8 year follow-up.

Objectives Of This Lecture:

  1. Participants will become familiar with the biology behind Partial Extraction Therapy (PET).
  2. When to consider PET in full arch therapy and classification.
  3. Participants will also learn how utilize a digital workflow to manage the prosthetic space in single and full arch cases.
